Welcome to Patuxent Cardiology Your First  Visit Please bring this information with you on the initial visit: A current updated list of all your medications and the dosage you are taking.  This includes all curent medications, non-prescritpion drugs and herbal supplements for review.  It is preferred if you can bring the prescription containers. A name of your primary care physician as well as other doctors participating in your care. This will enable us to maintain communication with your doctors. If you have had a recent EKG, blood work done and any reports or test results from you past medical history  Your most recent insurance cards and photo ID Copay and Referral (if applicable) Please do not hesitate to contact our office should you have any questions   INSURANCE We participate with most insurance companies. If your insurance requires a referral, you are responsible for obtaining a referral authorization from your primary care physician (PCP) for each office visit. The referral form must be given to our receptionist upon arrival, as well as your most recent insurance card and driver’s license. Please bring your insurance card(s) with you so we can verify that we have the correct information to bill your insurance company. Patients with co-pay requirements must pay at the time of their visit. If you are not prepared to pay, you may be asked to reschedule your appointment. We accept cash, Visa, MasterCard, and personal checks for payment.   REFERRAL If your insurance carrier requires you to have a referral, you must bring a copy with you. Without the referral, you may be asked to reschedule your appointment.   CANCELLATION / RESCHEDULING Please call  410-535-3612 if you need to cancel of reschedule your appointment, we would appreciate at least a 24-hour advance notice  for any appointment cancellation.
